Continuing Health Care

NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. In order to receive NHS CHC funding individuals have to be assessed by Clinical Commissioning Groups (CCGs) according to a legally prescribed decision making process to determine whether the individual has a ‘primary health need’.

The North East and Cumbria Learning Disability Network has an extensive range of work programmes that are delivered across secondary care, primary care, community care and tackling broader health inequalities.

We are proud to support our colleagues working in primary care across the region with a range of projects to help improve the experiences, health and health outcomes of people with a learning disability when using primary care services.
